Adding Survey Questions

Quick steps:

  1. Click the Lessons tab.
  2. Click on the survey you wish to add questions to.
  3. Click on the Add Question link in the toolbar.
  4. Click on Section Header to provide directions, or click on a question type link.
  5. Use the text fields, drop down menus and check boxes to select optional settings.
  6. Click the Save button when done.

Navigate to the Lessons tab and click on the survey you wish to add questions to (if you have just added a survey, you will already be where you need to be). Click the Add Question link in the toolbar.

You will be presented with several options. The first choice, Section Heading, is not a question type, but a place to provide directions or graphics for a group of questions that follows.

Adding Questions:

Select the link for a question format (Text Box, Text Area, Checkbox, Check List, Drop-down List, Option List, or Likert Scale). The following options vary depending on which type of question you choose:

  • Question: Enter the question text. You may use HTML tags, if desired.

  • Display Mode: Choices are Normal Display or Compact Display, which mainly affect how many lines will be taken by the question choices.

  • Choices: Depending on the type of question you choose, this item may or may not appear. This field allows you to enter a list of choices or options to be provided when the question is presented.

  • Add, Edit, Delete, Up, Down: These items on the Choices toolbar let you edit and rearrange the answer choices.

  • Copy, Paste: Let you copy the choices to paste into another question.

  • Selected/Unselected: For Checkbox or Check List question types, you can decide whether any or all boxes already will be checked by default when the student sees the question.

  • Advanced Settings
    If you select the Advanced radio button in the toolbar, additional settings are available. Advanced options include:

  • Field ID: You can fill in a descriptive label for your question here. This is useful particular if you wish to export data to another program such as Excel and need to identify fields.

  • Image URL: You can use this field to upload an image which will appear above the question text. Click Add, then in the window that pops up, click Browse to find an image on your computer, then click OK. Alternatively, if you want to display an image that is already on another web server, simply type or paste in the complete URL of the image (e.g. http://www.msu.edu/images/msu_logo.gif). You may also add images (of equations, for example) throughout your question text using HTML (see related topics).

  • When you are finished, click Save.
