Surveys can be useful to collect student input about the progress of a class, assess student attitudes on a relevant issue, or assess what the students know about a particular topic. The mechanisms for creating a survey are very similar to those for a quiz.
Creating a Survey "Shell":
Navigate to the folder in the Lessons tab where you wish to create the survey. Click Add Content in the toolbar, then click Survey to enter the Survey editor. Give your survey a descriptive title and, optionally, a subtitle and directions. Use the various tabs to set permissions. Use the Submission tab to set the maximum number of attempts that students are permitted to take the survey (Max Attempts). The default is "1," but the range is unlimited choices between 1-10 on a drop down menu.
